I could not tell you exactly, look for a Polo 6R IND (not 6C GTI) with factory LED headlights (team-bhp forum), in its Autoscan you will have the answer you are looking for.
The 7E0 BCM Max will be compatible as long as the parts, AFS unit and self leveling sensor, belong to the PQ25 platform, and ...

The 6R maxidot cluster (GTI, blue GT) will not fit into the 6C housing, perhaps you can have a look at the Seat Toledo cluster, that should fit in the 6C housing with some adaptations to the housing and PCB.
