Has anybody done one of these for an insurance policy?

im stuck on a part which asks to state who carried out the work done i.e fitting of coilovers and what their qualifications are.

I fitted these myself but i am not a mechanic and have no relevant qualifications.

whats the best thing to do?

Also should i state the price of the modifications when i bought them?

I have filled one out for my last car.

If you carried out the work just say that. I'm sure you wont be the only person to carry out their own work.
