Issues starting and running

Sometimes it will start straight up and run fine (usually on a morning) but sometimes it will struggle to start and take multiple tries. Also, when this happens it dies immediately after starting or sometimes it will die after it revs above 2000RPM. Also sometimes the engine dies while driving just after I change gear or when I stop at lights or when I accelerate reasonably fast.
Often the only way to get it running is to keep the starter motor on (key fully turned) to start it back up again as soon as it tries to die - after it does that a few times it runs fine. If I just try to keep restarting it normally it can take 5 minutes.
Also, the 'glow plugs' light occasionally flashes while I'm driving but I've read that this doesn't necessarily mean an issue with the glow plugs.
Any idea what could be causing this?
